The Health Declaration Form: Why It Must Be Complete and Accurate
The least popular but most important part of a health insurance application is the health declaration form (sağlık beyan formu). The question list looks long, some questions seem irrelevant and many people rush through it. Yet this form is the basis on which the insurer decides whether, and on what terms, to cover you. A small detail left out today can decide whether a treatment is paid years later. For newcomers to Türkiye, it also matters that the form covers your medical history abroad, not just treatment received here.
What is the declaration for?
Insurance is the sharing of an uncertain future risk. Before issuing a policy, the insurer assesses that risk by asking about your health. Your answers support three decisions: whether the application is accepted, whether specific conditions are excluded, and how the premium is set.
A declared condition does not always lead to a negative outcome. The insurer may issue the policy as it is, exclude only that condition or offer cover with an additional premium. Which route is taken depends on the insurer’s assessment and the policy terms.
What the form usually asks
- Conditions diagnosed in the past or currently
- Previous operations and hospital admissions
- Medication used regularly
- Ongoing treatment, and planned tests or operations
- Height, weight and some lifestyle questions
- Pregnancy
- Separate health details for each family member added to the policy
The last point is often missed. If your spouse or children are being added, their health history matters as much as yours.
Possible consequences of an incomplete declaration
An undeclared condition usually comes to light when a related claim is made. While assessing the claim, the insurer may review hospital records and doctors’ notes; if the complaint existed before the policy started, problems begin. Depending on the policy terms and legislation, this can lead to the claim being refused, the cover being changed or the policy being cancelled.
Bad intent is not required. A test result you thought unimportant, a treatment years ago or occasional medication can all be relevant, so answer each question according to its scope, not your own view of what matters.
Example: a forgotten past treatment
A policyholder leaves out a stomach complaint for which they briefly took medication in the past, thinking it trivial. Some time after the policy starts, similar symptoms return and further tests are needed. Reviewing the claim, the insurer finds the old records showing the complaint existed before the policy. Had it been declared, the insurer might have excluded it or offered an additional premium, but the policyholder would have known from day one what was covered. That certainty is the real value of an honest declaration.
Practical tips for completing the form
- Take your time: Fill it in when you are calm, with old reports to hand if possible.
- When in doubt, declare: If unsure whether something should be included, declare it and add an explanation.
- Check your records: Turkish records such as e-Nabız and medical files from your home country help refresh your memory.
- Use the explanation fields: Note when the condition appeared, whether treatment finished and your current status.
- Think about each family member separately.
- Keep a copy of the signed declaration with your policy documents.
If a new diagnosis, test or treatment arises between applying and the policy being issued, tell your agency or the insurer too. Online declarations confirmed by a link deserve the same care as a signed paper form.
After the declaration, and how we help

If the offer includes an exclusion or additional premium, read it in the special conditions and ask about anything unclear, so you know in advance which costs would be yours. Conditions that arise after the policy starts are generally not re-declared and stay within cover while the policy is renewed without a break, although a new insurer may ask for a fresh declaration if you switch.
